Membership Application

To be affiliated with the Carolina Heartland Chapter of USA Dance,
make sure to enter Carolina Heartland Chapter # 6092 on the application form.

There are two ways to apply for membership in USA Dance and Carolina Heartland:
  1. Send Application and Check by US Mail
    To join USA Dance and Carolina Heartland, please download, print, and complete the USA Dance Application Form available by clicking on this link
    Mail Application & Check to:

    Southampton PA 18966-0126

  2. Join Online and pay by Credit Card
    3. Recreational and Social dancers may also join online using a credit card at this link to the USA Dance, Inc web-site.

Please note: you don't have to be a member before you come out for a dance!
